Naming a Beneficiary
When naming a primary and contingent beneficiary, provide:
- Full name
- Address
- Social Security number
- Relationship to you
- Date of birth
- Distribution percentage
In most states, benefit payments cannot be made directly to a minor. If a minor is named, proceeds may be held under their name and earn interest until they turn 18.
